HBOT Before Surgery: The Four-Week Window Most People Miss
/If you have surgery scheduled in the next month, there's a tool most surgeons aren't telling you about. It won't replace your surgery. But it can change how well you heal from it — and the window to use it is closing faster than you think.
I'm Dr. Jason Han, doctor of physical therapy. I run HealthFit Physical Therapy & Chiropractic in Pasadena, where we focus on recovery and longevity. In this article, I want to walk you through something we talk about with patients every week: how hyperbaric oxygen therapy (HBOT) can help your body prepare for surgery, why the weeks before your procedure matter more than the day of, and how to think about it honestly — without the hype.
"It's Just a Knee" — and Why That Mindset Costs You
Maybe it's your knee. Maybe it's a nose job. Maybe it's a shoulder repair or a hip replacement. The way most of us talk about surgery, it sounds small. "It's just my knee." "It's just a quick procedure."
But every one of us is quietly hoping for the same thing: that we heal well, that we bounce back fully, that we get back to our lives. Nobody schedules a surgery hoping to heal slowly.
Here's the mistake most people make. They think recovery starts the day after surgery. It doesn't. It starts weeks before.
Surgery is a controlled injury. Your surgeon cuts, repairs, and closes — and then your body has to heal that wound. And the tissue that heals best is tissue that walks in already strong and well-supplied with blood. Think of it like training for a fight. You don't get ready the morning of. You prepare for weeks. The changes that actually matter for healing take time to build. That's why the three to four weeks before surgery is the window. Miss it, and you've left real recovery capacity on the table.
Why Kids Heal Faster Than You Do
Let me ask you something. Why do kids heal faster than adults?
Scrape a kid's knee and it's gone in days. We get older, and the same scrape takes weeks. It's not your imagination — it's your metabolism.
When you're young, your cells are energy factories running at full speed. As we age, that engine slows down. Your cells make less energy, and repair takes longer. That energy has a name: ATP. It's the fuel your cells burn to do every job, including healing.
And here's the key. The single biggest input for making that energy is oxygen. Good oxygen in, more energy out, better healing.
Now, we can't change Father Time. But in this age of biohacking, we can find smart ways to work around it. We can't make your cells young again — but we can give them more of what they need to do their job. And the most powerful lever we have is oxygen. That's exactly what hyperbaric oxygen therapy does.
What HBOT Actually Does Before Surgery
Inside a hyperbaric chamber, you breathe oxygen under increased pressure. That pressure dissolves far more oxygen directly into your blood plasma — under pressure, your plasma can carry up to ten times more dissolved oxygen than it does at sea level. That's not a supplement and it's not a pill. It's just oxygen, physics, and the right pressure.
In the weeks before surgery, that does three things that matter:
1. Tissue oxygenation. Oxygen dissolves into your plasma and reaches tissue that struggles to get enough through normal circulation — the exact tissue your surgeon is about to work on.
2. Inflammation regulation. Surgery triggers a big inflammatory response. Research suggests HBOT helps your body manage that response instead of letting it run wild.
3. Blood vessel readiness. Repeated sessions encourage your body to build new small blood vessels — a process called angiogenesis. More vessels means more delivery routes for everything healing requires.
And if you're wondering whether this is some fringe idea — it's not. Doctors use hyperbaric oxygen in hospitals for some of the most serious cases there are: flesh-eating bacterial infections, wounds that won't close, and carbon monoxide poisoning that starves the brain of oxygen. If it's trusted medicine when the stakes are that high, it makes logical sense that it can help your body prepare for a planned surgery.
I'll be straight with you, though. HBOT won't make your surgery unnecessary, and it's not a guarantee. But early research on pre-surgical HBOT is promising — some studies report fewer complications and shorter recovery times. I think of it as stacking the deck in your favor before you ever reach the operating room.
One Chamber for Everyone? That's the Wrong Approach
Here's where a lot of clinics get it wrong. They have one chamber, and they put everyone in it.
We don't do that. At HealthFit, we use three pressure levels for three different situations:
Soft chamber, 1.3 to 1.5 ATA. (ATA just means atmospheres of pressure.) At this gentle range, your oxygen levels can climb 400 to 600 percent. For general prep, energy, and an easy on-ramp, it's a great place to start.
Hard chamber, 2.0 ATA. This is the range most of the published surgical research lives in — roughly 1.75 to 2.4 ATA. At 2.0, your oxygen concentration can climb up to 1,200 percent. That's the level we reach for when the stakes are higher and the tissue needs a stronger push.
Which one is right for you depends on your procedure, your health history, and your timeline. That's not a guess — that's a conversation. We call it your Pre-Surgical Optimization plan.
HBOT Is the Centerpiece, Not the Whole Picture
One more thing most people miss. HBOT is powerful — but it's one tool, not the whole toolbox.
The patients who heal best treat the weeks before surgery like an optimization window. Oxygen in the chamber. Protein and nutrition to give tissue its building blocks. Gentle movement to keep circulation strong. Sleep, because that's when your body does its repair work. HBOT is the centerpiece; the rest is the frame around it. That's how we build a pre-surgical pathway — not one therapy, but a coordinated week of preparation.
The Bottom Line
Surgery is a controlled injury, and your body heals best when it walks in prepared. Healing comes down to energy, and energy comes down to oxygen. HBOT helps you prepare in three ways — oxygen, inflammation control, and blood vessel readiness. We can't stop the clock, but we can give your cells what they need to heal like they're younger. And the window to do this is the three to four weeks before your date. Not the day of. Before.
Ready to Talk It Through?
If you have a surgery on the calendar and you're wondering whether there's still time, reach out. We'll look at your timeline honestly and tell you whether pre-surgical HBOT makes sense for you — and if it doesn't, we'll tell you that too. Because this was never about a procedure or a protocol. It's about you, and helping you heal the best you possibly can.
